2. Pennsylvania Child Abuse History Check

3. FBI Affidavit of Compliance

If you have not lived in PA for 10 consecutive years:

1. Pennsylvania State Police – Access to Criminal History

2. Pennsylvania Child Abuse History Check

3. FBI Background Check – Fingerprints

***REFUND POLICY-

• Written refund request submitted to the Director PRIOR to the close of registration= 100% refund of registration cost - $5 for Team Snap Fees

• Written refund request submitted to the Director after registration closed but prior to the first practice.= 50% refund of registration cost -$5 for Team Snap Fees

• Written refund request submitted to the Director after the first practice 0% refund

All goals and other equipment located at Upper Saucon Community Park and Hopewell Park labeled with SLS are the sole property of Southern Lehigh Soccer Club and are only to be used by SLS players, SLS teams, SLS coaches and SLS affiliated trainers.

*Trainers, organizations and those not associated with SLS do NOT have permission to use SLS equipment.
